Who can refer?...

Referral can be made easily and quickly by a most health and social care professionals such as:

• Social Workers
• Care Managers
• Adult Social Care Teams
• Children and Family Social Care Teams
• Community Teams for Adults with Learning Disabilities
• Child Complex Health and Disability Teams
• Mental Health Teams
• Health Visitors
• GPs and other Health Professionals 

Referrals are also welcome directly from service users, carers or other family or extended relatives. Referrals can include self funding arrangements or funding through any of the arrangements under personalisation.
